    Description

    Medical ephemera:
    US Treasury NARCOTICS ORDER FORM
Series 1936 narcotics order form, under the 1914 narcotic regulations. Pharmacies had to order their opium, cocaine, and such using these forms.
This particular form is the second copy, the original probably buried in a government warehouse somewhere in case someone needs to check on a 1939 sale of morphine sulphate to a drug store in Camden, SC.
    Condition:
Shows little handling, carbon still readable even after 80 years
    .
Money back upon return if not satisfied or any problems with shipping within the US. Due to changes at the US Postal Service, we can no longer guarantee a refund on international mail service failure to deliver.  Thanks for looking.
